Section 407 of Title 17 of the United States Code authorizes the Copyright Office to collect the personally identifying information (PII) requested through this form. PII is any personal information that can be used to identify, contact, or trace an individual, such as names, addresses, and telephone numbers. The purpose of collecting the information on this form is to track, consider, and respond to requests to initiate a serial subscription for mandatory deposit. By providing this information, you agree to the routine use of it to establish and maintain a record of requests. The Copyright Office uses this information only to consider requests to initiate mandatory deposit via subscription and may share the information with the Library of Congress. The Office will not share it with other parties unless required by law or regulation. If you do not provide the information requested, the Office cannot process the electronic request to begin a serial subscription for mandatory deposit. You may make the same request on paper by following the instructions in 37 C.F.R. § 202.19(d)(2)(xi). Absent a grant of special relief, the owner of the copyright or of the exclusive right of publication in the United States may be subject to a fine if the requirements of section 407 are not met.
