Old Cable Statement of Account Forms

An important message for Cable and Satellite Operators on Filing Fees: Effective January 1, 2014, pursuant to the Satellite Television Extension and Localism Act of 2010 (STELA), which granted authority to the Copyright Office to establish fees for the filing of statements of account (SOAs) under the section 111, 119 and 122 statutory licenses, the Office now assesses filing fees for ALL SOAs for current, past and future accounting periods. For details see the Federal Register, November 29, 2013 (78 fr 71498). Please be advised that the filing fee is deducted before the royalty payment is credited; thus the omission of the appropriate filing fee will result in an underpayment of royalty fees. Please remit the royalty fee and filing fee in one EFT payment.

For semiannual accounting periods beginning July 1, 2010

SA1-2 Short Form — For use by cable television systems whose semiannual
gross receipts are less than $527,600

SA3 Long Form — For use by cable television systems
gross receipts are $527,600 or more

For semiannual accounting periods beginning January 1, 2010

SA1-2 Short Form — For use by cable television systems whose semiannual
gross receipts are less than $527,600

SA3 Long Form — For use by cable television systems
gross receipts are $527,600 or more

Note: Effective October 1, 2006, all statutory license royalty fee payments must be made via electronic funds transfer (read more).

For semiannual accounting periods beginning July 1, 2005

SA1-2 Short Form — For use by cable television systems whose semiannual
gross receipts are less than $527,600

SA3 Long Form — For use by cable television systems whose semiannual
gross receipts are $527,600 or more

For semiannual accounting periods beginning July 1, 2000

 SA1-2 Short Form - For use by cable television systems whose semiannual gross receipts are less     than $379,600 for semiannual accounting periods beginning July 1, 2000 and ending June 30, 2005.

SA3 Long Form - For use by cable television systems whose semiannual gross receipts are $379,600     or more for semiannual accounting periods beginning July 1, 2000 and ending June 30, 2005.

For semiannual accounting periods beginning January 1, 2000

SA1-2 Short Form - For use by cable television systems whose semiannual gross receipts are less     than $292,000 for semiannual accounting periods beginning January 1, 1990 and ending June 30,     2000.

For semiannual accounting periods beginning January 1, 1998

SA3 Long Form - For use by cable television systems whose semiannual gross receipts are $292,000     or more for semiannual accounting periods beginning January 1, 1998 and ending June 30, 2000.
